How does Cloudbeds PMS select the language of the confirmation emails?

Why this default status on Cloudbeds PMS does not apply to OTA reservations?

Each OTA has their own reservation settings and new reservations will receive an automatic confirmation for this third party. Access the OTA extranet directly to change the reservation status and inform your guests.

Will I be able to send a confirmation email to the guest when changing the reservation status?

Yes. When changing the reservation status to Confirmed , you will be asked whether to send a confirmation email to your guest:
